Both child and parent/guardian must be present at the in-person registration. Registration is NOT complete until the participant attends one of the in-person paperwork turn-in dates, regardless of whether you made a full or partial payment on-line.
Please bring the following items with you to in-person registration:

2020 Participant Contract and Parental Consent Form

2020 Physical Fitness & Medical History Form

Birth Certificate - Please bring a clear copy of your child's Birth Certificate.

Report Card - When final report cards are issued in June - Please scan and email a copy to playeragent@lccpw.org or make a copy of it and bring it to equipment distribution
